ADMISSION REQUIREMENTS:
Please comply the following documents prior to admission:
• Doctor's referral slip or Doctor's Admitting order from our accredited doctors, to wit :
- Jose Coruña Jr., MD
- Reagan Villanueva, MD
- Benson Go, MD
• Medical Certificate together with all laboratory results of the following:
- Chest PA, CBC, UA, HBsag, RBS, ECG and RAT
Note: please submit all these to CDOCH-House of Hope medical officer -in charge, OONA AGUILAR,MD. at JRBGH OPD every Tuesday and Thursday from 8:00am to 12:00 noon.
• Case study Report from CSWD Social Worker
- Qualifying client may bring the following upon admission :
✓ mat/banig, pillow and blanket
✓ Plastic cup for personal use
✓ Segregated plastic plate
✓ soap and shampoo
✓ Toothpaste with toothbrush
✓ Basket for toiletries
✓ Pail (balde) and dipper (kabo)
✓ Personal belongings and clothings
✓ Maintenance medication if any
